How Did You Hear about This Rotation?
The following questions are to help us get to know you.
Name a personal quality or life experience that you feel will augment the diversity of the unique community at HEART-IM.
Comment on an experience you had (or would like to have) in activism regarding medicine, medical education, domestic or international policy, social change or any other area that interests you.
If you have studied or practice any healing modality (e.g. meditation, tai chi, yoga, qi gong, homeopathy, herbs, body work, energy healing, etc) please describe the place that they hold in your life currently.
If you have not had exposure to any, no worries! There will be plenty of experiences at HEART-IM for you!
Our month at HEART-IM will be a transformative experience. In addition to our formal learning sessions, we would like to provide each participant with the opportunity for peer teaching. If you were to hold a short workshop, what would you consider hosting? Briefly explain your vision for a workshop and why you feel drawn to share?
Sample topics include, but are not limited to, healing modalities (meditation, yoga, herbs, etc), hobbies (knitting, juggling), activism (specific causes, methods of protest), creative expression (music, writing workshop, art).
Tell us why you feel drawn to participate in HEART-IM (e.g. How might a month of personal reflection serve you a the this time? What do you expect to address within yourself during a month of personal reflection? What do you expect from the HEART community? What do you hope to come away with?)
Please upload a copy of your CV.
